Sequel to planned/unplanned review- Community Setting:
- When a client has a planned or unplanned review and it leads to a service in a community setting
Sequel to planned/unplanned review- Nursing & Residential Setting
- When a client has a planned or unplanned review and it leads to a service in a residential or nursing setting
Sequel to request for support
- This is the sequel identified as the result of an initial screening/assessment with the client/representative before any type of support is provided. This encompasses decisions to by-pass short term interventions and provided long term services immediately, as well as issuing equipment or signposting to universal or voluntary sector services.
Sequel to Short Term Support to Maximise Independence(existing client)
- Of existing clients who have received 'Short Term Support to maximise Independence' a breakdown of what followed the period of short term support
Sequel to Short Term Support to Maximise Independence (new clients)
- New clients where the sequel to a request for support was 'Short Term Support to maximise Independence' a breakdown of what followed the period of short term support
