A stakeholder has queried the accuracy of the MODS definition: a trigger factor would normally be understood as something that causes a change in health or other circumstances, rather than a signal that a change has taken place. Recording trigger factors against risk would help people understand what needs to be avoided/managed in order to prevent the risk from happening.
